Raise a concern

• This form is to be used to officially document your concern in writing.
• All concerns will remain anonymous unless a request for a response is stated. If you would like a response, please include your contact details in your submission.
• All concerns will be reviewed within 10 working days, if you have requested a response, you will receive one within this time frame.
• A copy of the completed concern form and resultant written statement will be held and filed by Oxygen College in addition to being documented and followed through using the concerns register.
